Description
A complete set of structured, ready-to-use HTML assessment and planning tools built on NeuroHub Community’s Six-Point Framework For Supporting Autistic People. Designed for mentors, support workers, counsellors, and practitioners who work one-to-one with Autistic and neurodivergent people.
Most clinical and support tools weren’t built with neurodivergent people in mind. They flatten complexity into checklists, treat diagnosis as the whole picture, and leave practitioners improvising the parts that matter most — how someone’s sensory world connects to their mental health, how executive function shapes their daily reality, how identity and relationships interact with crisis. The Six-Point Framework exists because that gap is too important to leave unfilled.
This toolkit puts the full 6PF into your hands as three self-contained, professional-grade planning tools. No login. No subscription. No software to install. Open them in any browser, complete them with your client, export a polished PDF or copy a plain-text record into your case management system. Built for the reality of practice.
Tool 1 — The 6PF Global Assessment Record
The starting point for any new client relationship. The Global Assessment Record maps the whole person across all six domains of the Six-Point Framework — Sensory Processing, Executive Function, Identity and Self-Understanding, Relational Ecosystem, Mental Health, and Strengths — building a rich, holistic picture that no single-domain assessment can provide.
Each domain includes structured guidance for mentors, fields for strengths and challenges, 1–10 wellbeing ratings, and space for safeguarding notes and priorities. It’s designed to be completed collaboratively — with the client leading wherever possible — and it generates a full plain-text record ready for your notes system at the click of a button.
Use it for initial assessments, periodic reviews, or any point where you need a shared, structured understanding of where someone is across their whole life.
Tool 2 — The 6PF Burnout Recovery Planning Tool
Autistic burnout is not the same as general stress or exhaustion, and it can’t be addressed with the same tools. The Burnout Recovery Planning Tool is built specifically for the reality of neurodivergent burnout — a framework that identifies what in each domain is contributing to the load, what’s protecting against it, and what the smallest genuine next steps look like.
The “lilipad” approach at the heart of this tool asks practitioners and clients to think in terms of steps achievable on a difficult day — not aspirational goals that crumble under pressure, but genuinely tiny moves that build momentum without demand. Each domain includes space for burnout contributors, protective factors, recovery supports, lilipad actions, and wellbeing indicators to monitor over time.
The tool also includes an integrated recovery plan section — a whole-person synthesis across all six domains — so that individual domain work connects into a coherent direction of travel.
Tool 3 — The 6PF Substance Use Harm Reduction Plan
Substance use among neurodivergent people is common, frequently functional, and persistently misunderstood by mainstream support frameworks. This tool takes a non-judgmental harm reduction approach, grounded in the 6PF, that treats substance use as something to understand before it’s addressed — asking what the substance does for the person, in which contexts, and how the six domains of their life interact with that use.
The tool covers a full substance use baseline, a structured 6PF domain review with harm reduction strategies mapped to each domain, a detailed warning signs section co-created with the client, and a formal escalation risk assessment that considers the risk of frequency increasing, substances changing, and patterns becoming entrenched. It closes with a harm reduction commitments and safety plan — including safer use practices, named support people, external services, and an agreed response protocol if warning signs appear.
This is a tool that takes the person seriously without minimising risk — which is exactly what good harm reduction practice requires.
What all three tools share
All three are self-contained HTML files — no server, no account, no data leaving the device. They open in any modern browser on any computer and carry the full NeuroHub branding, the phoenix logo, and the rainbow gradient header that signals to clients and colleagues alike that this work is grounded in a coherent, professionally developed framework.
Each tool exports directly to a well-formatted PDF via a single button, prints cleanly for paper records, and generates a plain-text summary that pastes straight into any case management or clinical notes system. Domain-colour coding, mentor guidance callouts, and wellbeing rating scales are consistent across all three, so moving between tools as a client’s needs evolve feels intuitive rather than jarring.
They’re built to be used collaboratively — as artefacts you complete together with the person you’re supporting, not forms you fill in about them afterwards. That distinction matters. These tools are part of the session, not the administration that follows it.
This toolkit is for practitioners who already believe neurodiversity-affirming practice requires better infrastructure. These are the tools to build it with.
